
Making Your Appointment
Our staff members will try their best to schedule your treatments and physician visits at your convenience, based on availability. If you would like, your ongoing visits can be scheduled in advance.To schedule an appointment, please call Dr. Memon or Dr. Siddiqui at 815.756.5255 or Drs. Bhate at 815.756.4722.
Your First Appointment
When you come to The Cancer Center for the first time for your doctor’s visit, we ask that you arrive 15 minutes prior to your appointment time to complete forms.
Please bring the following items to your first appointment:
- Medical records - If tests were completed at a facility other than Kishwaukee Community Hospital, please be sure to have the following:
- Lab and treatment information to date
- Pathology slides and reports
- Chest x-rays and reports
- CT scans, MRI scans, and reports
- Surgery reports, if appropriate
- Insurance Card(s)
- List of medications with doses you are currently taking
- Download and print forms to complete prior to your first appointment
We encourage you to discuss what to bring with the person who schedules your appointment for you. You will be asked to verify insurance coverage and instructed to secure a referral from your primary care physician, if necessary.
Visits to the Center
Each time you arrive, please check in with the receptionist at the desk and please have a seat in our comfortable reception area. If you are seeing Drs. Bhate, you will be directed back to Illinois Regional Cancer Center’s reception area. A staff member will call your name when it is time to come back for your physician visit or treatment.Interpreters
Staff is available to provide interpretation services for our Spanish-speaking patients. When the Spanish interpreters are not available, or assistance with other languages is needed, we can use foreign language interpreters through a telephone system. We also have a service for sign language to help communicate with you if you are deaf or hard of hearing.
